Make a difference in long-term care as a Nurse Practitioner with Scarborough Health Network. This permanent, part-time role focuses on clinical care for residents, enhancing health outcomes through a collaborative approach.

As a Nurse Practitioner on the Nurse-Led Outreach Team, you will provide vital clinical support to residents in Long-Term Care homes. Key responsibilities include working with healthcare teams to prevent hospitalizations, enhancing care quality, and supporting transitions between facilities. This role requires a blend of clinical expertise, strong communication skills, and a passion for patient-centered care.

Key Responsibilities:

• Deliver clinical care to Long-Term Care residents

• Collaborate with healthcare professionals for better outcomes

• Reduce hospital admissions for patients with complex needs

• Provide assessments and interventions for chronic illnesses

• Facilitate training for Long-Term Care staff

Requirements:

• Master’s Degree in Nursing or Nurse Practitioner

• Current registration with the College of Nurses of Ontario

• 3 years of Nurse Practitioner experience

• Completion of controlled substances education

• Basic Cardiac Life Support certification
