Join Humber River Health as a Registered Nurse in the Home Hemodialysis Program, focusing on patient engagement and education in a part-time role. This position requires home visits and a commitment to improving patient outcomes through collaboration.
In this permanent part-time role, you will be responsible for comprehensive patient care and enhancing the quality of hemodialysis programs. Your work involves clinical assessments, training patients and families, and coordinating with interdisciplinary teams to facilitate effective treatment transitions.
Key Responsibilities:
• Conduct patient assessments and provide tailored education
• Engage in community and home visits to support patients
• Work collaboratively with interdisciplinary teams for patient care
• Document interactions and clinical data in Meditech
• Participate in home support nurse rotations as needed
Requirements:
• Must be registered with the College of Nurses of Ontario
• Minimum of 2 years of nephrology and hemodialysis experience
• Current BLS Provider certification
• Proficiency in Microsoft Office and Meditech tools
• Valid driver’s license with reliable transportation required
