Take a leading role in diabetes care as a Nurse Practitioner focused on integrated community programs and endocrinology. This advanced practice role is rooted in direct patient care and education.
You will play a crucial role in managing interdisciplinary patient care for complex diabetes cases at both the RADAR Clinic and within inpatient settings. Key Responsibilities:
• Coordinate care and manage diabetes patients in various clinical settings
• Partner with healthcare professionals to optimize patient outcomes
• Facilitate diabetes education and support services
• Contribute to evidence-based research in diabetes therapies
• Assist in program development and evaluation efforts
Requirements:
• RN (EC) license and Nurse Practitioner Certification
• Must possess Certified Diabetic Educator (CDE) credential
• Minimum three years’ experience in diabetes education
• Strong clinical knowledge in managing diabetes care
• Excellent communication and leadership skills
