In this permanent, full-time position, you will coordinate care for a diverse clientele, ensuring each patient is suitably educated and followed up in a home setting.
Your role will involve collaboration with a multidisciplinary team, optimizing care through technology and experience in nephrology.
Key Responsibilities:
• Educate patients and families on home dialysis practices
• Monitor treatment safety and adequacy for care continuity
• Coordinate patient appointments and follow-ups
• Participate in on-call rotations for added patient support
• Apply research-based interventions in clinical practice
Requirements:
• College of Nurses of Ontario registration required
• Minimum 2 years of nephrology experience needed
• Knowledge of ORN data collection preferred
• Strong leadership and assessment skills
• Must possess a valid driver's license and vehicle access
