Become a key part of Humber River Health as a Registered Nurse in the Home Hemodialysis Long-Term Care Satellites. Deliver vital care and support for chronic kidney disease patients.
This permanent part-time position involves direct patient interactions, effective treatment coordination, and collaboration with a dynamic team. You will champion evidence-based clinical practice and support patients through independent dialysis education and care management.
Key Responsibilities:
• Coordinate clinical activities for dialysis patients
• Conduct assessments and provide educational support
• Facilitate communication among care team members
• Participate in quality improvement initiatives
• Document clinical data accurately for reporting
Requirements:
• Registration in good standing with the College of Nurses of Ontario
• At least 2 years of nephrology nursing experience
• Competency in clinical documentation systems
• BLS certified, CNeph certification preferred
• Ability to adapt in a collaborative healthcare environment
