Become a Nephrology Nurse at Humber River Health focusing on Home Dialysis. Enhance patient independence through education and clinical support in a home environment.
This full-time role is instrumental in the Home Hemodialysis Unit, aiding chronic kidney disease patients. You'll lead assessments and patient education, ensuring they develop the skills for successful independent dialysis. Close collaboration with a multidisciplinary team and managing home visits are key components to delivering comprehensively safe and effective care.
Key Responsibilities:
• Assess and educate patients on home dialysis therapies
• Coordinate patient appointments and follow-ups
• Assist in monitoring quality indicators for treatment adequacy
• Facilitate communication with dialysis vendors
• Participate in continuous quality improvement initiatives
Requirements:
• Registered with the College of Nurses of Ontario
• 2 years nephrology experience with hemodialysis
• BLS certification essential; CNeph certification preferred
• Strong assessment and teaching abilities
• Valid driving license needed for home visits
