Registered Nurse for Adult Visiting Program

Provide advanced patient care as a Registered Nurse in the Adult Visiting Nurse program with VHA in East Toronto. The role includes evening and weekend shifts focused on specialized nursing care and patient support.

VHA Home Health Care seeks a dedicated Registered Nurse to enhance our Adult Visiting Nurse program. In this position, you will deliver advanced clinical care, manage care plans, and ensure quality patient experiences. Key responsibilities include performing clinical tasks like wound care and IV management while supporting mental health and coordinating care across the community.

Key Responsibilities:

• Assess and coordinate patient care effectively

• Evaluate care interventions through reassessment

• Maintain high standards of patient documentation

• Inform supervisors of client condition changes

• Participate in on-call duties and team meetings

Requirements:

• Registered and in good standing with CNO

• Valid First Aid or CPR certification

• Excellent communication and documentation skills

• Commitment to continuous learning

• Valid Ontario driver’s license and reliable vehicle
