Key Responsibilities:

• Conduct health assessments and physical exams

• Order and interpret diagnostic tests swiftly

• Develop evidence-informed care plans collaboratively

• Manage comprehensive acute and chronic health conditions

• Ensure timely referrals and care transitions

Requirements:

• Master’s degree in Nursing preferred

• 3-5 years nursing experience in community or primary care

• Current NP registration with the College of Nurses of Ontario

• Experience working with marginalized populations

• Proficiency in trauma-informed care and EMRs
