Nurse Practitioner-Extended Class

Position: Nurse Practitioner-Extended Class Needed
Unison Health and Community Services is seeking a Nurse Practitioner-Registered Nurse, Extended Class for a part-time role in Toronto. Focus on patient care through assessments and treatment in a supportive community setting.

This part-time, contract position involves working 21 hours weekly and requires a keen commitment to client health. You will diagnose and manage common health issues, provide oversight and education to staff, and collaborate with other health professionals. Engaging with diverse communities, you’ll support initiatives from an inclusive and equity-focused framework.

Key Responsibilities:

• Diagnose and treat common health problems

• Collaborate on patient care with primary care providers

• Supervise clinical staff and student placements

• Develop work goals with your supervisor

• Report safety incidents and assist in client care

Requirements:

• Extensive experience in a community-based healthcare setting

• Registration in Ontario’s Extended Class required

• Proven ability to work independently and as part of a team

• Excellent skills in triage and clinical judgments

• Knowledge of local community health issues
