VHA is looking for dedicated professionals to provide comprehensive nursing services within the community. The role involves working bi-weekly weekend evening shifts, delivering advanced clinical care to adults 18 and older. You will manage medication, wound care, mental health, and other complex needs, with an emphasis on quality and client autonomy.
Key Responsibilities:
• Assess and monitor client health conditions
• Evaluate and adjust care plans based on reassessment
• Maintain documentation standards as per regulations
• Participate in team meetings and on-call rotations
• Enhance client care through continuous quality improvement
Requirements:
• Registered with the College of Nurses of Ontario
• RPN diploma or RN degree from a recognized institution
• Valid First Aid or CPR certification
• Valid Ontario driver's license and reliable vehicle
• Effective communication and documentation skills
Provide essential nursing care and build meaningful relationships with clients at VHA Home Health Care in East Toronto.
