As part of a multi-disciplinary team, you’ll tackle clinical challenges in long-term care, streamline hospital discharges, and minimize unnecessary emergency visits. Your expertise will contribute to leadership in patient care and collaboration across healthcare settings, enhancing the overall care experience for residents.
Key Responsibilities:
• Provide clinical assessments and interventions for LTC residents
• Work with teams to strategize successful discharges
• Enhance LTC staff competency through training
• Apply best practices and current research into clinical settings
• Improve communication for care transitions
Requirements:
• Advanced degree in Nursing or Nurse Practitioner certification
• Current CNO registration as an NP required
• Minimum 1-3 years experience as a Nurse Practitioner
• Strong leadership and communication abilities
• Ability to travel within Ontario with valid driver’s license
