This casual position requires availability for various shifts including days, nights, and weekends across multiple sites. Utilizing your geriatric nursing skills, you will assess patients, manage medications, and communicate effectively with health teams.
Your role also involves patient education and collaborating with community partners to enhance care.
Key Responsibilities:
• Conduct comprehensive geriatric assessments for inpatients and outpatients
• Manage medication titration and follow-up testing
• Develop and evaluate safety plans of care
• Provide family education and support recommendations for referrals
• Collaborate with an interprofessional team for effective care planning
Requirements:
• Current Registered Nurse or RN Extended Class registration
• Minimum five years geriatric nursing experience
• Canadian Nurses Association Gerontology Certification
• Experience with case management and cognitive assessments
• Proven communication and leadership skills
