Palliative Care Registered Nurse in Ontario

VHA Home Health Care seeks dedicated RNs and RPNs for its Palliative Care team in the Central region. You'll coordinate nursing services for individuals and families, adhering to community nursing standards. This role also involves collaborating with interdisciplinary teams to deliver high-quality end-of-life care.
Key Responsibilities:
• Assess and coordinate patient care plans
• Monitor ongoing patient conditions and evaluate care interventions
• Maintain documentation per College of Nurses standards
• Participate in on-call rotations for client support
• Collaborate with external care teams and health professionals
Requirements:
• Current registration with the College of Nurses of Ontario
• Valid First Aid or CPR Certification
• RPN diploma or RN degree from accredited institution
• Experience in acute care or community nursing
• Reliable vehicle and valid Ontario driver’s license
Provide essential palliative care with VHA, ensuring personalized support for clients at home.
