Lead patient flow operations at Humber River Health, North America’s first digital hospital. This part-time role focuses on ensuring timely patient admissions, transfers, and discharges.

In the role of Patient Flow Manager, you will facilitate patient movement within the state-of-the-art Wilson site in North West Toronto. Your expertise will be essential in coordinating admissions and clinical resources while managing quality assurance during day and night shifts. Well-honed critical thinking skills are needed to address flow issues and optimize operations effectively.

Key Responsibilities:

• Coordinate patient admissions, transfers, and discharges

• Monitor analytics related to patient care timeliness

• Collaborate with staff to improve system processes

• Lead after-hours clinical services as night manager

• Track and facilitate discharge workflow activities

Requirements:

• Registered Nurse with current CNO membership

• Baccalaureate Degree in Nursing required

• Minimum 5 years clinical experience

• At least 3 years in leadership within acute care

• Master’s Degree preferred or actively pursuing
