Registered Nurse Adult Visiting Program

Join VHA Home Health Care as a Registered Nurse in West Toronto, providing specialized in-home care for adults. Deliver advanced clinical support, such as wound care and medication management, while enjoying flexible scheduling.

In this role, you will support the Adult Visiting Nurse program with evening shifts on weekends. Ideal candidates will deliver high-quality, client-centered care to individuals aged 18 and over in their homes. You will collaborate with other healthcare professionals, while forming meaningful relationships with clients and their families.

Key Responsibilities:

• Assess and coordinate comprehensive care for clients

• Monitor and evaluate ongoing care plan effectiveness

• Ensure patient safety and quality in care delivery

• Maintain accurate documentation per CNO standards

• Participate in team meetings and on-call rotations

Requirements:

• Valid RN or RPN with good standing from CNO

• Ontario driver's license and reliable vehicle required

• CPR certification and wound care knowledge preferred

• Experience in Acute Care or Medical-Surgical nursing

• Excellent communication and documentation skills
