Position SummaryClinical Nurse Specialist (Nurse Navigator), Cardiology Your goal in this position is to provide continuous resource for patients and their families by maximizing emotional support, coordination of care and education across the entire continuum of care for patients with congestive heart failure (CHF). You have expert clinical skills in the assessment, planning, implementing, evaluating, and documenting the delivery and coordination of care for the CHF patients and their families.
You have excellent communication and interpersonal skills. You have excellent leadership, collaboration, decision making, problem recognition, and problem-solving skills. You have excellent change management skills and also project management skills. You have evidence of program design and resources development skills. You also have excellent practice program development, project management, and report writing skills. You have a high level of initiative, work independently, and can effectively lead project teams to achieve outcomes.
This position supports the hospital’s academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation. This position contributes to the shared responsibility of ensuring health and safe policies are followed to provide for a healthy environment for patients, staff, physicians, volunteers and families.
- Map present and future trends to create and sustain a seamless and continuous process for patients undergoing CHF care
- Assume a leadership role in influencing clinical practice and patient outcomes
- Collaborate with the broader health care team and community partners regarding complex and challenging patient care situations
- Integrate research into practice to ensure best practice standards are maintained. Create, utilize, and/or disseminate new nursing knowledge
- Embrace technology and innovation to better support patients, families and the program
- Support patient navigation throughout care journey:
Assess, plan, implement, evaluate and document the delivery and coordination of CHF pathways during the entire episode of care - Educate patient and caregiver(s) throughout the CHF pathway on complex cardiac conditions, medications and monitoring requirements
- Identify and coordinate community resources to meet supportive care needs of patients throughout CHF pathway
- Provide emotional support and guidance to patients and their caregiver(s) coping with congestive heart failure
- Ensure timely access to appropriate diagnostic tests and referrals to specialized clinical professionals across the episode of care
- Demonstrate and ongoing commitment to continuing professional development
- Demonstrate knowledge of evaluation methodologies
- Assume a leadership role in influencing clinical practice and patient outcomes and collaborates with the broader health care team and community partners regarding complex and challenging patient care situations
