Humber River Health in Toronto is seeking a Nurse Practitioner to lead care within the Nurse Led Outreach Team (NLOT). The NP will provide clinical leadership in LTC settings, collaborating with physicians and inter-professional teams to optimize hospital discharge and reduce unnecessary ED visits.
