Registered Practical Nurse: Acute Care Focus

This role requires a collaborative approach to providing person-centred care in the C6 Community Programme, focusing on Internal Medicine, Gynecology Oncology, and Plastic Surgery. Your skills will be essential in assessing care needs, implementing interventions, and evaluating patient experiences. Continuous professional growth and commitment to ethical care are expected.

Key Responsibilities:

• Identify patient and family needs for effective care

• Perform comprehensive assessments and modify care plans

• Implement and evaluate quality evidence-based interventions

• Facilitate patient education and discharge planning

• Commit to quality improvement and reflective practice

Requirements:

• Current RPN registration with the College of Nurses of Ontario

• Heart and Stroke Foundation CPR certification

• Preferably 6-12 months of relevant acute care experience

• Recent participation in the Sunnybrook Advanced Assessment Course is an asset

• Critical thinking and teamwork skills
