The South East Grey Community Health Centre (SEGCHC) is a primary care facility located in Grey County. We have four sites located in Dundalk, Markdale and Chatsworth. The SEGCHC is dedicated to meeting the broader needs of the communities in which we serve through the development and implementation of primary care, health promotion and community development best practices.

We are currently seeking (1) one full-time (37.5 hours per week) Primary Care Nurse Practitioner for a 14-month contract (with possibility of extension) starting at the end of October 2026 at our Dundalk location. Reporting to the Director of Health Services and working as part of a Community Health Centre multi-disciplinary team, the NP’s responsibilities will include:

  • Strong collaboration and communication skills while working with an interdisciplinary team including physicians, other nurse practitioners, registered practical nurses, medical administration, allied health staff and community staff.
  • Initiative, sound clinical judgement, and confidence in working independently.
  • Flexibility, adaptability, and resilience in a dynamic healthcare environment.
  • Respect, empathy, professionalism, and commitment to fostering an inclusive and welcoming environment for patients and colleagues.
  • Experience working with diverse, vulnerable, or underserved populations would be an asset.
  • Must have current registration with and be a member in good standing with the College of Nurses of Ontario, with a minimum of 3-5 years’ progressive primary care experience.
  • Experience in chronic disease management strategies and program development is necessary.
  • Advanced proficiency in the use of computers and various software applications including Microsoft Office and electronic medical records systems (Practice Solutions would be considered as an asset).
  • Experience in and/or a strong understanding of the CHC model of care.
