As a Care Assessment Coordinator, you'll perform clinical assessments for prospective residents in hospitals and homes. This role requires a Registered Practical Nurse (RPN) certification and blends clinical skills with sales support through collaboration with Lifestyle Consultants. Prior experience in geriatrics is a plus for enhancing your impact.
Key Responsibilities:
• Conduct clinical assessments across hospital and community settings
• Collaborate with Lifestyle Consultants for resident transitions
• Provide timely assessments to aid in urgent situations
• Act as a resource for appropriate community matching
• Build relationships with hospital staff and community partners
Requirements:
• Registered Practical Nurse (RPN) in good standing
• Experience in geriatrics or retirement living preferred
• Strong assessment and communication skills
• Self-motivated and organized in work approach
• Passionate about supporting seniors and families
