The Palliative Care team at Hennick Bridgepoint Hospital provides specialized support for individuals whose care needs shift toward comfort, function, and quality of life. Featuring 24 private rooms, we provide specialized care for patients during the advanced stages of terminal illness or chronic disease. The care we provide is guided through collaborative goal-setting, ensuring patients are active participants in shaping the support they receive.

• Practice independently and accountably within the RPN scope of practice, adhering to CNO standards and Sinai Health policies
• Identify changing patient needs to initiate timely consultation and collaboration with Registered Nurses and the wider interprofessional team
• Monitor and document the progress of clinical care plans and outcomes, adjusting strategies as needed to optimize treatment goals and patient safety
• Participate in the mentoring of
