This nursing position requires an experienced professional with a minimum of 2-3 years in mental health or substance use environments. You will be instrumental in assessing and planning care that aligns with individual client needs and the overarching goals of the withdrawal management program. Joining a dedicated team, you will focus on empowering clients through education and effective interventions, while managing and ensuring the safety of their care.
Key Responsibilities:
• Conduct thorough assessments and document care needs
• Create and adapt client care plans collaboratively
• Facilitate group and individual therapeutic interventions
• Ensure medication safety throughout the management process
• Engage with inter-disciplinary teams for comprehensive care
Requirements:
• Registered Practical Nurse in good standing with Ontario’s College
• 2-3 years of experience with substance use and mental health
• Understanding of trauma-informed care and harm reduction
• Excellent communication and documentation skills
• Ability to work both independently and collaboratively
