Flemingdon Health Centre is looking for an experienced Registered Nurse to join its Unify care team. In this regular full-time role, you will provide comprehensive primary care, coordinate services, and deliver health education. Evening and weekend hours are expected, supporting a dynamic healthcare environment that focuses on community health.
Key Responsibilities:
• Provide comprehensive primary care within RN scope
• Coordinate care across health services and external providers
• Deliver client education during assessments and groups
• Conduct telephone triage and follow-up on results
• Support outreach program development based on community needs
Requirements:
• RN in good standing with the College of Nurses of Ontario
• Bachelor of Science in Nursing (BScN)
• More than 1 year of primary care experience
• Strong assessment and community development skills
• Flexibility for evening and weekend hours
