UHN is seeking a dedicated Clinical Nurse Coordinator to ensure seamless care for patients with suspected or confirmed CTEPH. You will be responsible for coordinating diagnostic tests, managing patient communications, and facilitating collaborations with healthcare teams. This role contributes significantly to improving patient outcomes in a research-oriented clinical environment.

Key Responsibilities:

• Coordinate CTEPH patient assessments and management

• Triage and schedule consultations efficiently

• Oversee diagnostic test coordination for patients

• Facilitate case reviews and treatment planning

• Provide educational support to patients and caregivers

Requirements:

• Bachelor’s degree in Nursing required

• Current registration with the CNO

• Relevant clinical experience preferred

• Excellent communication and organizational skills

• Ability to thrive in a fast-paced setting
