Patient Flow Manager Role

Elevate patient experiences at Humber River Health as a Patient Flow Manager. This part-time role is crucial for managing the clinical flow of patients within our digital hospital.

You will be instrumental in ensuring timely patient admissions and discharges while utilizing clinical data to manage the use of resources efficiently. This role requires a balance of analytic skills and leadership capability in a dynamic, fast-paced environment. Reporting to the Program Director, you will implement strategies to improve patient care outcomes.

Key Responsibilities:

• Oversee patient admissions, transfers, and discharges

• Analyze patient care data for prompt responses

• Manage staffing to ensure patient safety

• Collaborate on systems improvements with departments

• Ensure after-hours crisis management within clinical areas

Requirements:

• Registered Nurse in good standing with CNO

• Bachelor’s Degree in Nursing required

• At least 5 years of clinical experience

• Minimum 3 years in a leadership role

• Master’s in related field preferred
