Carefirst RN​/RPN Position in Home Care

Carefirst is hiring a Registered Nurse (RN) or Registered Practical Nurse (RPN) for their dedicated Home Care Team in Scarborough. Be part of an initiative that supports seniors and adults with health challenges.

Our non-profit organization is looking for a committed RN/RPN to fulfill essential responsibilities in client assessments, case management, and clinical support.

This role offers the chance to make a meaningful difference in program delivery while collaborating with a passionate team serving over 15,000 clients annually.

Key Responsibilities:

• Assess client needs and deliver case management support

• Serve as a clinical consultant for complex health cases

• Submit Initial and Interim Client Assessment Reports

• Attend meetings for continuous quality program improvement

• Offer advice to the Infection Prevention and Control Committee

Requirements:

• Nursing or Practical Nursing degree/diploma required

• At least 3 years of community healthcare experience

• Active member of the College of Nurses of Ontario

• Current First Aid & CPR certification

• Strong communication and organizational skills
