The Diabetes Education Community Network (DECNET) is a partnership among South Riverdale Community Health Centre, East End Community Health Centre, Access Alliance Multicultural Health and Community Services, and Michael Garron Hospital. Working within the context of community health principles and practices, DECNET provides diabetes education support and services to adults with pre-diabetes and Type 2 diabetes in East Toronto. Particular emphasis is placed on increasing access for mental health, hard to reach, complex, and marginalized populations and members of our community.
The Registered Practical Nurse (RPN) – Footcare – DECNET works as an integrated member of an inter-professional team within the DECNET team of SRCHC. The RPN performs assessments, develops client-focused treatment plans, provides health education/counselling, and other clinical activities. The RPN works within the scope of practice as outlined by the College of Nurses of Ontario (CNO) and must consult with an RN, NP, MD, or Chiropodist to manage complex client or medical presentations.
This position will work collaboratively with other interdisciplinary team members, health care professionals / service providers and clients in the identification and resolution of health issues and integration of care, and strives for excellence in client centered care and education. Working onsite is required.
- Complete intake and assessments as necessary e.g. health histories, social determinants of health, functional and family status, and physical assessments such as wound assessments, both in order to establish a plan of care within RPN scope of practice and to connect clients to additional services
- Provide footcare and treatment of foot-related health conditions in a supportive, therapeutic, and preventative means in order to attain/maintain optimal function of the client
- Perform foot/lower leg assessments, provide foot/lower leg treatment, health education and other clinical activities according to the College of Nurses of Ontario Professional Standards and within the scope of practice for RPN's and Advanced Foot Care Nurse Certification
- Work independently, and under the guidance of Medical Directives, or in consultation with other SRCHC primary care providers or other relevant inter-professional team members as appropriate
- Educate and advise a wide range of clients, based on the client’s goals and practical ways in which they can improve their health by adopting healthier foot care/lower leg habits, facilitating self-management strategies, incorporating clients’ health goals, culture and income
- Work collaboratively with prescribing primary care providers (PCP's), and other health care providers to provide optimal client care
