Join VHA Home Health Care as a full-time RN/RPN, focusing on the Child and Family program in Toronto Central. Provide essential nursing care to young patients, enhancing their health and well-being at home.

Key Responsibilities:
• Assess and plan individualized care for clients
• Monitor and evaluate the effectiveness of care interventions
• Maintain high documentation standards as per regulatory bodies
• Participate in standby/on-call rotations for client support
• Attend team meetings and collaborate with peers
Requirements:
• Good standing with the College of Nurses of Ontario
• Nursing diploma or degree required
• Experience with wound care or medical-surgical nursing preferred
• Strong communication and documentation capabilities
• Valid driver's license and reliable vehicle for travel
