Job overview
Reporting to the Associate Director of Care (ADoC) of the assigned Resident Home Area (RHA) the Registered Nurse serves Kensington Health’s LTC home (i.e. the Gardens at Kensington Health) to support and advance quality of care, safety and the experience of the people we serve.
This role will be guided by the legislation and regulations governing LTC in Ontario, the policies and procedures and best practices in accordance with the Standards of Practice of the College of Nurses of Ontario.
Responsibilities
Coordinates the Registered Practical Nurse (RPN) and Personal Care Attendant (PCA) team on the assigned RHAs to provide effective care for all residents consistent with each resident’s care plan.
Assists RPN in completing clinical assessments for residents who present as unstable and/or who present with a change in health condition (e.g. clinical deterioration, palliative, and/or end of life).
Assists RPNs with new resident admissions /transfers/readmissions/discharges.
Assists RPNs with residents’ emergencies and /or other emergencies as they arise.
Follows up on critical results and ensures on-call/attending physician are Nurse Practitioner are notified.
Supports RPNs to identify and manage clinical risks (e.g., responsive behaviors, falls, skin integrity, etc.).
Oversees the development of an interdisciplinary plan of care for each resident with resident and family input.
Ensures the resident and family are aware of resident care changes (e.g., changes in health condition, changes in medications, safety incidents, etc.).
Serves as a clinical resource person in identifying and solving resident care problems.
Evaluates, in collaboration with the rest of the interdisciplinary care team, the care provided for residents Communicates with and supports families in their involvement and participation, as needed.
Assist in planning assignments for RPN and PCAs as directed by management.
Promotes an environment in which the health care team works co-operatively and supportively.
Documents in the health record, as needed, as per the organization’s policies, procedures and the College of Nurses of Ontario’s standards of practice.
Notifies the Associate Director of Care/delegate of any safety incidents, risk issues or other matters as per the organization’s policies and procedures.
Promotes the Residents’ Bill of Rights.
Promotes person-centered practices and approaches in all resident encounters and decisions.
Promotes an environment in which the health care team works co-operatively and supportively.
Participates in the Long-Term Care Home’s programs (e.g. Occupational Health and Safety Program, Quality Improvement, Fire Safety, WHMIS).
Participates in required training and education and engages in quality improvement activities as needed.
