In the Nurse Practitioner role, you will enhance resident well-being by assessing and managing acute and chronic conditions. This position collaborates closely with Medical Doctors and the interdisciplinary team, emphasizing advocacy for residents and engaging families in care decisions. Your expertise will lead clinical initiatives addressing various needs, including palliative care and dementia management.

Key Responsibilities:

• Assess and diagnose resident health conditions and treatments

• Collaborate with MDs and teams for quality care

• Lead clinical discussions in resident care conferences

• Advocate for residents' best interests and needs

• Interpret trends to create educational programs for staff

Requirements:

• One year of experience in geriatric or long-term care nursing

• Registration with College of Nurses of Ontario

• Leadership experience considered an asset

• Certifications in gerontology or palliative care preferred

• Knowledge of Long-Term Care Act and related regulations
