In this pivotal role, you will lead the coordination and effective delivery of care to Community Members. Your leadership will inspire teamwork, accountability, and clinical excellence within your department. Key Responsibilities:

• Provide day-to-day leadership and support to the Wellness Team

• Manage performance and development of team members

• Conduct Wellness Assessments and implement Care Plans

• Support compliance and quality improvement initiatives

• Administer medications and documentation accurately

Requirements:

• Registered Practical Nurse with the College of Nurses of Ontario

• At least 5 years' experience in a similar setting

• Demonstrated leadership or supervisory experience

• Strong clinical assessment and decision-making skills

• Must have CPR/First Aid certification
