Nurse Practitioner for Senior Care

West Park LTC is hiring a Nurse Practitioner for a contract role that offers flexibility for both part-time and full-time work. You will collaborate with healthcare teams to optimize care for medically complex residents and prevent avoidable hospital transfers. Your leadership will help foster an environment of person-centered healthcare.

Key Responsibilities:

• Conduct comprehensive assessments of health conditions

• Diagnose and manage both acute and chronic issues

• Prescribe and oversee medication management

• Engage in care planning with family members and staff

• Provide clinical leadership and educational support
Requirements:

• Valid NP license in Ontario and CNO registration

• OHIP billing number, with independent contractor comfort

• A minimum of 3 years NP experience in geriatrics preferred

• Experience in chronic disease and dementia care

• Strong skills in communication and team collaboration
