Position Summary

The Registered Nurse at Stem Health is a core clinical team member who translates the promise of our care model into the lived experience of every member who walks through the door. You will work at the full scope of your RN practice, supporting our physician team across primary care, preventive health, annual health assessments, and episodic care in a fast-moving but deeply intentional clinical environment.

This is a role for a nurse who takes pride in clinical precision, communicates with warmth and professionalism, and understands that in a membership-based clinic, the quality of the nursing encounter is as important as the quality of the medicine. You will be a critical part of a small, high-performing clinical team reporting to the Director of Clinical Operations, starting September 22, 2026.

Key Responsibilities Clinical care and nursing assessment
  • Provide evidence-informed nursing care within RN scope of practice and College of Nurses of Ontario (CNO) standards across primary care, preventive care, annual health assessments, episodic care, and chronic disease management.
  • Conduct comprehensive nursing assessments, health histories, vital signs, medication reconciliation, clinical screening, and member intake establishing a thorough and accurate clinical picture prior to each physician encounter.
  • Recognize and respond to abnormal clinical findings, changes in member condition, or urgent presentations escalating promptly and appropriately to the attending physician or clinical leadership.
  • Support annual health assessments and executive preventive health programs with clinical professionalism and the level of discretion and attentiveness that Stem Health's membership model demands.
Clinical procedures and IV therapy
  • Perform clinical procedures within RN scope and demonstrated competency, including venipuncture, specimen collection, injections, immunizations, ECGs, wound care, and IV access.
  • Administer, monitor, and discontinue IV therapies in accordance with authorized orders, Stem Health protocols, and applicable clinic policies maintaining clinical vigilance throughout each infusion.
  • Maintain competency across all assigned clinical procedures and proactively seek guidance or additional training when encountering unfamiliar presentations or equipment.
Patient education and closed-loop care
  • Provide clear, accessible patient education regarding treatments, medications, procedures, preventive health recommendations, and follow-up plans ensuring every member leaves with a confident understanding of their care.
  • Coordinate referrals, diagnostic investigations, laboratory requisitions, specialist follow-up, and clinical recalls managing the full care loop so that nothing falls through and every member receives timely, complete follow-through.
  • Communicate results, updates, and between-visit information to members in a manner consistent with Stem Health's standard of care warm, professional, and thorough.
Clinical collaboration and interdisciplinary teamwork
  • Collaborate closely with physicians, medical office administrators, diagnostic staff, and allied health providers to deliver seamless, coordinated care that reflects the full clinical picture of each member.
  • Participate in clinical huddles, case briefings, and team communications to ensure alignment on care priorities, member needs, and clinic-level operational readiness.
  • Contribute a nursing perspective to interdisciplinary discussions and support the integration of nursing assessments and observations into each member's care plan.
Clinic operations, documentation, and quality
  • Maintain accurate, timely, and complete documentation for all assessments, procedures, interventions, medications, and clinical communications within the EMR meeting Stem Health documentation standards and CNO requirements at all times.
  • Maintain clinical room readiness, supply management, equipment function, and infection prevention and control (IPAC) standards ensuring a safe, well-organized, and consistently prepared clinical environment.
