As part of the Diabetes Education Centre, you will leverage your nursing expertise to assess and educate clients from diverse backgrounds. Your commitment to continuity of care and systemic advocacy plays a vital role in enhancing community health. You will also work to foster a supportive environment for clients and healthcare professionals alike.
Key Responsibilities:
• Conduct assessments and develop diabetes management plans
• Collaborate with dietitians and endocrinologists on patient care
• Utilize various teaching methods in group and one-on-one education
• Maintain communication with clients and healthcare providers
• Engage in policy development within clinical practices
Requirements:
• Current registration with the College of Nurses of Ontario
• Minimum 2 years’ experience in a Diabetes Education Program
• Certification as a Diabetes Educator
• Strong organizational and initiative-taking skills
• Proficiency with EMR systems
