As a Registered Practical Nurse in the 6G Medicine and Transitional Care unit, your primary responsibility will be delivering holistic nursing care while promoting wellness. You will work collaboratively with an inter-professional care team to ensure optimal patient experiences and outcomes through innovation and evidence-based initiatives.
Key Responsibilities:
• Advocate for patients' wellness and care needs
• Establish therapeutic relationships with patients
• Provide autonomous care for stable patients
• Collaborate with care team members effectively
• Coordinate patient care activities with clear communication
Requirements:
• Completed Practical Nursing Diploma Program
• Registered with the College of Nurses of Ontario
• Valid BLS/CPR Certificate
• One year of geriatrics clinical experience preferred
• Excellent English communication skills
