Key Responsibilities:
• Conduct assessments and monitor client health
• Create and implement customized care plans
• Ensure patient care meets accreditation standards
• Maintain accurate documentation practices
• Engage in supportive team collaborations
Requirements:
• Registered with the College of Nurses of Ontario
• Diploma or degree in nursing from an accredited institution
• Current First Aid or CPR Certification
• Community nursing experience is a plus
• Valid Ontario driver's license required
