Position: VHA Home HealthCare RPN Role

As part of the Hospital Partnership Program, VHA is hiring Registered Practical Nurses focused on post-hospital care for clients. Successful candidates will manage their schedules while delivering personalized home health nursing services. Responsibilities include conducting assessments and providing specialized care such as IV therapy while working collaboratively with healthcare partners.

Key Responsibilities:

• Create and monitor individual care plans for clients

• Conduct regular assessments and evaluate care interventions

• Provide wound care and chest tube management

• Engage in team meetings and support quality care initiatives

• Participate in on-call rotations as needed

Requirements:

• Must be in good standing with the College of Nurses of Ontario

• RPN diploma from a recognized college

• CPR or First Aid Certification necessary

• Experience in community nursing preferred

• A valid driver’s license and dependable vehicle required
