This role involves managing patient care for geriatric inpatients while coordinating with healthcare teams. Key Responsibilities:
• Manage care for geriatric and general medicine patients
• Collaborate on care planning with multidisciplinary teams
• Document patient care efficiently via computerized systems
• Engage in committees and project teams as required
• Provide mentorship for new hires and nursing students
Requirements:
• Valid RN registration with the College of Nurses of Ontario
• Required BCLS certification
• At least 2 years of medical/surgical nursing experience
• Excellent communication and organizational skills
• Understanding of equity and diversity principles in healthcare
Contribute to quality elderly care at Humber River Health and lead positive changes in patient support.
