Key Responsibilities:
• Provide direct client care within the expanded RN scope
• Educate and promote health within community groups
• Coordinate care with health providers, ensuring continuity
• Conduct telephone triage and home visits as needed
• Participate in audits and quality improvement initiatives
Requirements:
• Registered Nurse in good standing with the CNO
• Bachelor of Science in Nursing required
• Over 1 year experience in primary care settings
• Strong community engagement and teaching skills
• Flexible scheduling for evening/weekend availability
