Become a vital part of Carefirst’s Home Care Team in Scarborough as a Registered Nurse (RN) or Registered Practical Nurse (RPN). Impact your community by assessing and supporting client health needs.
This full-time position at Carefirst Seniors & Community Services involves a range of responsibilities from client assessments to crisis intervention and program evaluation. You will join a dedicated team serving a diverse clientele of more than 15,000 people, delivering quality health services in the community.
Key Responsibilities:
• Conduct comprehensive client care assessments
• Provide clinical support and consultation for healthcare providers
• Report on client assessments to service partners
• Collaborate on program planning and quality improvements
• Guide infection control practices effectively
Requirements:
• Degree or diploma in Nursing/Practical Nursing
• 3+ years in a healthcare setting
• Good standing member of Ontario’s College of Nurses
• Valid First Aid & CPR certification
• Proficient communication and leadership capabilities
