WELL Health Diagnostic Centres is the largest operator of independent diagnostic health facilities in Ontario. We provide cardiology, radiology, respirology, and sleep diagnostic services at 60+ accredited locations across the province.

Responsibilities
  • Conduct comprehensive health assessments, including medical histories and physical examinations in collaboration with cardiologists and Internal Medicine specialists.
  • Provide patient education and counseling regarding disease prevention, treatment options, and healthy lifestyle choices.
  • Monitor patient progress and adjust treatment plans as required.
  • Collaborate with physicians, specialists, allied health professionals, and community resources to ensure continuity of care.
  • Maintain accurate and timely clinical documentation within the electronic medical record (EMR) system.
  • Participate in quality improvement initiatives and support best practices in patient care.
  • Adhere to all regulatory, professional, and organizational standards.
  • Perform consultations and stress test supervision as assigned.
  • Perform all medical tasks delegated by the attending physician.
  • Ensure a safe, clean working environment and call for assistance when required.
  • Document patient care services by charting in patient records and recording vitals, history, and prescriptions.
  • Perform any other tasks assigned by the supervisor.
Qualifications and Experience
  • 0-2 years of related experience.
  • Registered Nurse Practitioner certification is required.
  • Member in good standing with the College of Nurses of Ontario.
  • Demonstrated knowledge and understanding of general medicine.
  • Ability to collaborate and communicate effectively with health professionals and patients.
  • Ability to function effectively in a clinical setting.
  • Familiarity with electronic medical records (EMR).
  • Competence in customer service, communication, teamwork, initiative, accountability, dependability, flexibility, and adaptability.
  • Computer literacy in MS Office and ability to research on the Internet.
  • Work effectively within a diverse community and show empathy toward patients and families.
  • Work independently or as part of a multidisciplinary healthcare team.
  • Commitment to continuing education and training.
  • Effective decision‑making and problem‑solving skills.
