In this essential role, you will work with an interdisciplinary team dedicated to improving the health of individuals who are homeless or under-housed. Through comprehensive nursing assessments and tailored care plans, you'll address acute medical needs while promoting health equity. This is an impactful opportunity to work in southeast Toronto where diverse community health issues are at the forefront of care.
Key Responsibilities:
• Assess, document, and address client health needs
• Collaborate on interdisciplinary care strategies
• Administer medications based on care plans
• Provide pre and post-operative patient care
• Engage in ongoing health education for patients
Requirements:
• Must hold R.N. Registration with the Ontario College of Nurses
• Preferred BScN or extensive nursing experience
• 3+ years in acute and mental health care settings
• Basic Cardiac Life Support certification required
• Understand trauma-informed care principles
