Position: Home Dialysis Registered Nurse Position

As a Home Dialysis Registered Nurse, you will deliver patient-centered care and implement individualized treatment plans.

Your role involves numerous assessment activities, health teaching, and family counseling, ensuring patients receive comprehensive support. Essential responsibilities include participating in weekly clinics, monitoring clinical data, and mentoring colleagues in nephrology practices.

Key Responsibilities:

• Plan and provide personalized care for home dialysis patients

• Perform assessments and document follow-up activities

• Educate patients and families on modality choices

• Analyze clinical data and apply critical thinking in care

• Participate in home dialysis clinics and follow-up care

Requirements:

• Current Registration with the College of Nurses of Ontario

• Recent Hemodialysis experience (3 years required)

• Baccalaureate degree in Nursing preferred

• Certification in Nephrology CNeph(C), preferred

• Valid driver's license and access to a vehicle
