As an RPN at Unity Health, you will collaborate with RNs and the Inter-professional team to provide comprehensive nursing care.
Your role includes assessing patient needs, contributing to individualized care plans, and documenting clinical outcomes. With a focus on patient-centered principles, you will support both patients and families through the rehabilitation process and beyond.
Key Responsibilities:
• Assess and evaluate patients’ health and care needs
• Develop and implement personalized care plans collaboratively
• Advocate for patients and families effectively
• Document care accurately in line with nursing standards
• Maintain patient independence and recovery support
Requirements:
• Practical Nursing Diploma completed
• Registered with the College of Nurses of Ontario
• Current BCLS certification required
• Experience with older adult patients preferred
• Strong communication skills needed
