Part Time Registered Practical Nurse

This part-time role emphasizes personalized care for clients post-hospital discharge, co-managing their health for 90–120 days. Key responsibilities include head-to-toe assessments and delivering specialized services such as wound care and IV therapy. Work will be day shifts with a weekend rotation, requiring reliable transportation and an Ontario driver’s license.

Key Responsibilities:

• Conduct comprehensive health assessments for clients

• Coordinate and evaluate personalized care plans

• Monitor and document changes in client conditions

• Maintain professional relationships with clients and families

• Participate in team meetings and on-call rotations

Requirements:

• Registered with the College of Nurses of Ontario

• RPN diploma from an accredited institution

• First Aid or CPR Certification

• Nursing experience in acute care preferred

• Valid Ontario driver’s license and vehicle
