Support older adults as a Nurse Practitioner at Oak Valley Health’s Reactivation Care Centre, applying advanced geriatric assessment and clinical decision-making skills.
This Nurse Practitioner provides comprehensive nursing services for older adults at the Reactivation Care Centre site of Markham Stouffville Hospital. Working within an interprofessional team, the NP supports reactivation for patients designated Alternate Level of Care. The position emphasizes evidence-based geriatric care, consultation, and patient-centred practice.
Key Responsibilities:- Provide comprehensive nursing services for older adults
- Promote health, prevention, treatment, rehabilitation, and support
- Deliver direct patient care and professional consultation
- Promote best practices in geriatric nursing care
- Collaborate on Alternate Level of Care reactivation
- Master’s Degree
- Current Ontario RN (EC) Nurse Practitioner registration
- Minimum five years’ current geriatric care expertise
- Advanced clinical knowledge and evidence-based practice skills
- Strong decision-making, communication, coaching, and leadership skills
