Deliver resident-centered clinical care as a full-time, on-site Nurse Practitioner at Humber Heights in Etobicoke, Ontario. Apply geriatric assessment expertise.
Schlegel Villages seeks a Nurse Practitioner to support quality resident care within its long-term care community. The role combines clinical practice, education, leadership, and quality improvement. You will guide interdisciplinary teams while supporting holistic care for residents and families.
Key Responsibilities:- Complete assessments and support diagnostic testing procedures
- Order, interpret, and communicate diagnostic testing results
- Lead resident-centered palliative care services
- Educate and support multidisciplinary Village teams
- Participate in clinical reviews and quality initiatives
- Current Nurse Practitioner registration with the College of Nurses of Ontario
- Experience completing comprehensive geriatric assessments
- Current Heart Save Level CPR certification
- Current AIS certification
- Experience leading, coaching, and mentoring healthcare professionals
