POSITION DESCRIPTIONThrough a collaborative team of physicians, nurse practitioners, registered nurses, allied health professionals, and administrative staff, the clinic will provide integrated, person-centred care that improves access to primary care, advances health equity, and supports better health outcomes for the communities it serves. This is an opportunity to be part of an innovative model of community-connected care that is helping shape the future of primary health care in East Toronto.
Providing primary health care, the Nurse Practitioner works collaboratively with other interdisciplinary team members, health care professionals/service providers, partners and service users. This position is required to work flexible hours, including evenings and weekends onsite, at partner locations, or through mobile work.
Working onsite is required.
