Support clients and families through home healthcare coordination at C-Care Health Services in North York. Work full-time on-site with care plans and assessments.
This Client Care Coordinator role provides support to clients and staff through proper communication and coordinated care. You will serve as the main contact for concerns about client health and condition. Responsibilities include in-home assessments, staff communication, and monitoring care documentation.
Key Responsibilities:- Assess clients in their homes as services require
- Update care plans according to client needs
- Discuss plans and changes with care staff
- Communicate concerns and significant events to families
- Review staff attendance, conduct, notes, and reports
- Active CNO RN or RPN license
- Current valid BLS certificate
- Effective verbal and written communication abilities
- Care coordination background is an asset
- Client management experience is an asset
Use your CNO nursing license, assessment skills, and communication strengths to coordinate client care across the agency.
