Deliver person-centred primary care as a Nurse Practitioner at South Riverdale Community Health Centre in East Toronto. Work onsite with flexible hours.
This Nurse Practitioner provides primary health care through an interdisciplinary, collaborative team model. The position supports integrated care, improved access, health equity, and better outcomes for communities served. Work occurs onsite, at partner locations, or through mobile work.
Key Responsibilities:- Provide primary health care to community members
- Collaborate with physicians and registered nurses
- Work with allied health professionals and administrative staff
- Coordinate with partners, service providers, and service users
- Support integrated, person-centred community care
- Nurse Practitioner qualification for primary health care
- Ability to work within an interdisciplinary team
- Availability for flexible hours, including evenings
- Availability to work weekends when required
- Ability to work onsite and at partner locations
